Lemaire1; 1Cetparp, Lille, France; 2Marseille, Marseille, France In this study independent of the industry, we prospectively assessed the efficacy and tolerance of tadalafil (TAD) in 91 consecutive patients aged more than 50 who visited our private practice for ED. 79 (87%) judged TAD improved their erections but only 58 (64%) were fully satisfied. Mild or moderate adverse events occurred in 22 (24%), including headache in 4%, vasodilation in 7%, gastralgia in 7%, and back or muscle pain in 11%.Among the 75 patients who had previous experience with sildenafil (SILD) and used it appropriately, the proportions of complete, partial or absent improvement of the erections were the same on SILD and on TAD. 37% preferred SILD and 47% TAD although 38% judged their erections stronger on SILD and only 27% on TAD. 20% elected to continue with SILD and 62% with TAD. 8% alternated the 2 drugs. TAD was effective in < 30 mn in 42% of the patients. 67% felt it was effective > 24 h including 23% > 48h. In this real world study, the efficacy and safety of TAD were therefore close to those reported in the pre-registration studies. The comparison with SILD may have been biased by selecting patients little satisfied with this drug. However it shows that some patients may prefer TAD though they feel SILD more potent, and that each drug may salvage failures of the other. The main reasons for preferring TAD were linked to its longer effect, which may exceed 48h in some patients and allows for more spontaneity. |
